QuestionPro - Research Access Forums

I created a poll and copied the html which is posted below. I am pasting it into a Dot Net Nuke (cms) website. The poll will not accept my choice.

Support says "I need to close the tag" but couldn't or wouldn't say anything else.

Here is the code, any ideas?
